Yes- I interviewed there last fall. (Sidenote- they have a nice clinic & nice profs) In order to apply to StonyBrook, you have to call them up and request that they send you a paper application. Give them your best, correct address to send it to, and expect to wait because it won't come immediately- most people I knew had to wait a couple weeks.
So, call them up first thing monday so you can get it soon.
