Success in clinical yrs depends on preclinical?

I'm just wondering how much it matters if I've been bumming around for the last couple of years and am about to begin my clinical years in medschool. Generally speaking, if you've been knowing just enough to pass preclinical med, would you look like an idiot on the wards? + Do you need to put in a monumental effort to get a good step 2 score?
