how useful are they in the long run? ie are they just to help get your name familiar to a certain med school? if you had to choose between a summer working at your current lab and doing a summer research internship, which would be better?

I guess programs have prestige because of the name and the selectivity.. but if you're working in your other lab for a long time and you get some long-term experience because of it.. then stay that course instead.

not really a big deal, i mean unless you present a poster at the end of the summer or something simliar

At your lab. It is highly unlikely that you will be able to have publishable work after the end of a summer program. Stick with your lab, where you do actually have a shot.
