Surgical Residency after a prelim year

Are surgical spots (pgy2) following a transitional year at a different sight few and far between? Are these surgical positions generally obtained a year in advance (concurrent with matching in a prelim. position). Also, how does being away for a few years post-transitional year effect your competitiveness in obtaining a surgical position (pgy2), assuming your doing something medically related.

A transitional year doesn't count towards surgery residency. A preliminary year will count, if you transfer into a categorical position.

I know several people who for one reason or another did a surgery prelim year. All of them transferred into pretty strong categorical positions.

What did you do while you were out of medicine? I'm going to guess that you'll probably have to start over, if you want to do surgery, even if your internship was a prelim surgery year.

I agree with MH on the fact that at TY won't get you far to get a PGY2 spot. It may even be a long shot to get a second year spot after doing PY a couple years ago. If you are interested, you may have to go through the normal matching procedures again. I wouldn't wait until after the match to try to find yourself a 2nd year spot, or else you may be gambling with another year of your professional life. Also, most places would probably expect that you would have had a very good reason to be gone from clinical medicine for a couple years, e.g. a death in the family, to take you on. Being that the attrition rate of a lot of surgical programs is relatively high, a lot of programs would have serious second thoughts about taking on someone who skipped out of a program after one year--they would definitely see it as a serious liability. However, due to the 80-hr rule, some programs that are short on residents may be interested just to fill the spot. Especially if you can spin your absence in a good way. I think that you could probably contact your previous program's program coordinator to see if they can throw your name into the PGY-2 pool
