Taking a Leave of Absence 6 weeks before the semester ends?

I am trying to decide whether or not to take a leave of absence from my post bacc. I have been struggling with severe mental health issues that I have sought help for and medications are helping but I still feel overwhelmed at times and unable to perform to my full potential. I have been going back and forth with this decision for about a month now but can't bring myself to just walk away. Unfortunately, there is only 6 weeks left in my semester and taking a leave of absence would likely push me back an application cycle but I also know post baccs do not have room for bad grades any semester so I am very unsure of what to do. Please help
