Some schools (Michigan, UW, Midwestern, Nova, Colorado) require microbiology for their program. I didn't see much on their websites about whether or not it needs to be from a 4-year. Do these schools care if its from a CC instead or a 4-year?

Nominally, no.

Yes. I know UW highly prefers that all pre-reqs be taken at a university. I can't speak for the others, but I would assume that they would want the same.

Do schools accept microbiology if its taken online (without the lab) from a four year institution?

I took a regular lecture Microbiology without a lab so I think it should count. Call the admission office to make 100% sure.
