Hey guys, please please help!
Taking Naplex in 1 day so desperately need your help!

1) what is the max dose of APAp? I thought it was 3.6g, but I think, if I am not mistaken, RX prep says its 4g...

2) Math: (I did 120 questions, and ASHP math.) So, the ASHP book asks the E value from melting points. I have not seen it in rx prep, nor in 120 SDN q-s. Do I need to study that?

3) LDL goals...RX prep gives the goal (unbolded) then the optional goal (bolded). Do I study optional goals then? (For example, High risk, CHD or CHD eqviv, Framington more than 20%, unbolded goal is less than 100. Optional goal, which is bolded, less than 70) I think at school we went by 70, but I dont remember anymore...

sorry, one more:
INR: if pt is stable on warfarin, do we check INR q month (according to the old CHEST guidelines) or q 4 months (according to the 2012 CHEST guidelines)
???
Why did the CHEST guideline had to change warfaring monitoring few months before my NAPLEX?:cry:

I guess another one:
Asthma...Are combos b2 agonist/steroid ( such as Symbicort and so on) considered resque inhalers? Patient is supposed to use it BID so, more than 2 times per week...

I am thinking it is not considered as an emergency inhaler, even though it has b2 agonist... Am I right?

No that is not a rescue/ emergency inhaler. The b2 agonist in these combo products are long-acting b2 agonist. Rescue ones are short-acting b2 agonist (ie. albuterol). Hope this helps.

Max dose for APAP is 4g .... for severe alcoholics 3g/d

e value will be given to you

Rescue inhalers are SABA's only... i.e Albuterol inh

Unfortunately, you have to know the new CHEST guidelines... And yes, we're in the same boat, my exam is on the 7th... :S

Good Luck for your exam
APAP dose = 10 -15mg/kg Q4-6 Hr Max 5 doses/day = approx 4 g
120 questions covers everything...except some biostat questions but as you are going for exam tomorrow so dont start anything new now.
LDL goal for high risk patients is always below 70. if you have option between 100 and 70 then go for 70 for high risk patients who have two or more conditions. (chd,dm)
INR for warfarin on stable patients as per new guidelines is 4 months.
B2 Agonist (albuterol) are rescue med in pregnancy and other conditions...

Also regarding E Values from freezing point ( i am not sure about melting point) are not so tough.check out chapter 11 from pharmaceutical calculations book (ansel and stoklosa).

Latest CHEST guidelines (9th edition) state that if INR is consistently stable, can consider INR testing frequency of up to 12 weeks (grade 2B).

What about taking any long acting formulation more than once daily?
Are there any drugs that are xl, er, sr, la and so on that is ok to take bid, or more than once daily?
I thought there was something, but I can't remember :(

you need to calm down...Don't freak out the day before your naplex, at this point you either know it or don't. You Know your stuff, get a good night sleep and stay calm and collected, you WILL be fine.

One thing I remember for sure is Wellbutrin..

Wellbutrin XL is once a day...
Wellbutrin SR is twice,
Wellbutrin IR is tid...

NAPLEX in few days OFFICIALLY in freak out mode !!! I tried my best and hope I will be okay :(

Just dont freak out if you make a mistake. Make yourself to move on and focus on the next questions!
That was the biggest challenge for me

lots of math! 120 SDN calc question is a must! Do it two or three times. RX prep was sufficient for me. No diabetes, few ID, stats NNT, lots of alzymers/parkinson...LOTS of math
