I had some questions about proper etiquette in replying to letters of rejection that will eventually be headed my way, so I did some research. Here is one proposal:
Dr. <XYZ>
<Director of Admissions>
<ABC Med School>
Dear Dr. <XYZ>,
Thank you for your letter of <date>. After careful consideration, I regret to inform you that I am unable to accept your refusal to offer me admission to your school.
This year I have been particularly fortunate in receiving an unusually large number of rejection letters. With such a varied and promising field of med schools it is impossible for me to accept all refusals.
Despite <ABC Med School>s outstanding qualifications and previous experience in rejecting applicants, I find that your rejection does not meet my needs at this time. Therefore, I will be entering the class of 2007 at <ABC Med School>. I look forward to seeing you then.
Best of luck in rejecting future applicants.
Sincerely,
<Your Name Here>
