My state of Nevada got a C- and that was generous. We got good marks for having a huge percentage of ER visits seen by board certed EPs. That's great but the state's population all lives in 2 urban areas where the community standard is BC EPs. We don't have rural populations served by ERs staffed by non BC EPs.

I wonder about some of the things the report cards looked at too. Preventive care is great but does it really impact me in the ED? I guess it could change my census or acuity. It is not an indicator of the quality of EM care provided in a state.

They were right on the money about overcrowding being a huge issue. I can attest to that.

Is this kind of report even helpful to anyone? Will low marks make the politicians fix anything? Will high marks make them reduce funding or give up on tort reform? What does the public think about it? Will my patients interpret this as meaning that I'm a C- doctor? Hmmm.

A camel is a horse built by committee. This report is not helpful because states are not homogenous. If you look at cities, where you get EM-trained docs and shorter transport times to the ED, it works out. In that same state with 300-400 miles to that city from a remote hospital, you don't get the same. The A and the D average to a C+.

Moreover, it's like using your shoe for a hammer and the edge of a butter knife as a flat head screwdriver. It works, but it CERTAINLY is not a best fit.

Anybody smart enough to make a difference is too smart to run for office. All you need is an hysterical lawmaker to jump all over this, MFing docs in general (and EM docs in particular), to appease "the people", and, ironically, even as we are the "front line", we're demonized as sucky, "C-" doctors. However, it's all light and sound and bluster, and I predict no change.
 
I tend to agree. NYC isnt going to fare the same as the rest of the state. It is interesting but doesn't seem to help much. 🙁
