Does anyone know if this test is worth doing? How does it compare to the AAMC/real thing?

Also, should it be done with the structure and timing of a real test, or should it be done just for practice?

Are you talking about the practice test that comes with the Official MCAT Guide sold by the AAMC? Because that practice test is actually an AAMC test of your choice (4-10). IIRC, the bundle costs $45.
 
yes, with my TPR package, i received "2009 Edition: The Official Guide to the MCAT Exam."
 
Just to clarify, there are problems in the Official Guide book. The AAMC also sells a package deal where you get the Official Guide, including the problems in it, AND online access to an AAMC FL. It sounds like you're just talking about the problems IN the book. In that case, just take the test as you normally would. Do NOT write in your book to somewhat simulate taking the test on a computer. You should take the problems IN the book towards the end of your schedule.
