I don't use a checklist, but I will note mood, affect, orientation, social interaction, ability to play interactively, creatively, and cooperatively, ability to tolerate frustration, speech, eye contact, response to boundaries or limits, activity level. I will also make note of any unusual behaviors that might be apparent. This is one area where it really helps to have lots of experience with kids to develop a baseline or range of normal basis for comparison.

Some of the old tests have such checklists. If your institution has an old test set, it might be worth it to look at them.
