Scenario: It is established that antiretroviral drugs help patients who have AIDS. It is understandable that take away an effective treatment and replace it with a placebo will cause harm to the patient.
Scenario 2: If I have a Drug A that is standard of care and Drug B for which I want to test efficacy then what if any concerns are there?
There will be no placebo in this trial because it would be unethical, but are there ethical concerns in replacing Drug A with Drug B where the efficacy is not known?
