Two electives in the same subspecialty?

I'm thinking of doing two electives in the same IM subspecialty. My thought was that I could excel in the second one (where I'd do it at a top program) having learned a lot from the first and get good LORs from there. But would it look bad on the application if I don't have a variety? Does it matter?

No, it won't look bad if that is what you are interested in.
 
I don't think that it is necessary to do that. You can get excellent LORs on your first go-round w/ a specialty. They don't expect you to know everything, they expect to see you learn while you are there. I think it would be best, from an educational perspective, to get as much exposure broadly as you can. If you choose one specialty, you will have your whole life to get to know that really well. It would serve you well to spend that one rotation somewhere else where you will gain knowledge and experience in that topic.
