Ok, so I had this raging fever a couple days ago, and I wondered, for fever-relief is it better to take two Tylenol every four hours as directed and would it make much of a difference to take one Tylenol every two hours after the initial two?

This may be some insane fever-induced reasoning, but I figure you need a certain concentration of acetaminophen in your bloodstream to get that anti-pyretic effect. But, if you took one every two hours after the initial two simultaneous caplets would you avoid the drop in concentration of acetaminophen in your blood?

Heh, anyone have any ideas? =P

I posted this on the allopathic forum as well.

If you're concerned about keeping the acetominophen in therapeutic range, then I would take two first as a loading dose then split the subsequent doses and dosage intervals to reduce the vacillations in concentration. (if you infinitely split the doses and intervals it would be the same as a continuous IV drip).

(i have to say it probably wont make one lick of a difference in practice)

Hey Pewl,

Hope I don't sound to preachy, but, generally, I think it's best to stick with the dosing guidelines listed on the medication packaging. They're put there for a reason. Especially for acetaminaphen, which has a very narrow therapeutic index (dosage used is very close to overdose range!) - overdosing will really destroy your liver.

So, keep with the recommended dose and suck on frozen sliced fruit or ice to bring down the fever.
Ciao! 😉

Acetaminophen has a very wide therapeutic window and sans alcohol is very unlikely to cause liver failure (via P450 induction and glutathione depletion). It is in fact one of the safest analgesics we have.

That's not true. You'd have to take like a whole bottle to do liver damage. For example, "extra strength" tylenol is just like double the normal dose.

I take ONLY 500mg "Extra Strength" caplets =D
