Is there a way to distinguish say a left-sided LMN lesion of XII vs a right-sided UMN lesion of XII? Both would deviate the tongue to the left. Also, what are some good neuro resources, ie. Practice questions, summaries, etc.

MRI

But seriously I don't think this distinction matters for most cranial nerves at the medical student level. The only cranial nerve lesion I can think of where upper vs lower mattered was the facial nerve.

Don't fasciculations and muscle wasting in the tongue help you differentiate between an UMN and LMN lesion on opposite sides?
