Yes I think you are pretty unique and should have a lot of options if you can write a strong personal statement to tie it together. Plus good reference letters.

Since you have no research experience, the only drawback I see is that the research powerhouses may not covet you but otherwise the sky’s the limit.

UCSF UCLA UCSD USC UCI Stanford Mayo to start. UChicago Columbia Vanderbilt. I’ll let @Faha weigh in on Harvard Yale Hopkins etc., as I have less expertise on reaseach schools.

Any school with a mission to serve the underserved would be good.

Some of the schools I listed might be considered research schools but you have enough other things to interest them.
