I'm hoping to reach out anyone that can help me improve my chances in securing a Psychiatry residency spot.

I went unmatched last season due to not having Step 3, and late scores from CS (in December)

YOG 2015, Step 1 (212) with 1 attempt, Step 2 (244), CS, and Step 3 (228) all passed first attempt.
Step 1, 2, and 3, were all taken within a 24 month period 2017-2019.


I have been utilizing my MBA in healthcare administration as a patient care coordinator and operations manager for a successful psychiatry practice.

I have made many improvements to the group during my time there, including those related to improving patient care and significantly increasing revenue.

Most recently I have been directly supervising nurse practitioners that I hired and contracted, along with support from their collaborating psychiatrists.

My training and experiences include:

1. A diverse medical, administrative and technical background.
2. Managing the administrative and clinical operations of a Psychiatry group.
3. Managing care coordination of patient cases with community resources including integration of chronic care management services.
4. I have experience working with crisis services, Medicaid and Medicare programs.
5. I have remained clinically active in my medical education by attending CME events and activities and have been actively involved in patient care since graduation.

During my time in private practice management I have fostered an atmosphere of improving care by:

1. Ensuring continuity of care with current and previous providers.
2. Linking with data exchange databases for up-to-date hospitalization and lab data.
3. Implementing a chronic care management program which utilizes appropriate scales for regular monitoring and follow up.
4. Encouraged the review of full available medical data in clinical decision making by making it available to providers and offering support when needed for mid level providers.
5. Developed a clinical educational program for PGY-1 and PGY-2 Family medicine residents that rotate at the group for their psych elective requirement.

So what's your application history? Any interviews? Sounds like your experience makes you a good administrative candidate, but don't see much in clinical experience.

I had 2 interviews last match after applying to 119 programs. I applied FM/Psych/ and some IM.
The programs I interviewed were 1st year accredited AOA programs- They offered positive feedback but did not match me.
I reviewed the new residents and these programs did not take any MD candidates, opting to stick with DOs

I have been doing externships and observerships in ACGME approved hospitals - I have updated letters from 2018 and 2019.
My most recent 2019 externships were 6 weeks each -
One was inpatient psych, with heavy exposure to ECT and substance abuse cases.
The other was in Infectious disease specialty, I worked closely with IM residents and followed closely with an infectious disease attending at his practice and his inpatient cases.

Both preceptors wrote LORs for me.

I also have a critical role in the group I manage: I screen new patients, administer surveys, follow up with patients on the phone regarding medication responses, coordinate their care through community resources and referral to appropriate services and specialists, and triage acute cases for physicians.

I have many examples, for one: most recently I had an established patient that was overtly manic show up at the clinic with no appointment. I was able to engage in communication with patient, put them in a low stimulated environment, and gather relevant information regarding patients mental state before any of the physicians were able to see them for an acute visit. During interview with patient and family members, med reconcilliation and review of I-stop, I was able to provide physicians with relevant information and discover that the manic state was likely brought on by a stimulant medication started by a PA at her PCP.

I think you need to come up with a base strategy. You're 4-5 years post-grad and I think that sticks out so you have to counter that. The fact you got very few interviews last time doesn't bode well for getting more this next round. If you're strategy is to go through the Match again I would suggest focusing on the residencies that are new and go heavy on family med and internal med with a sprinkling of psych.

I agree that the number interview last cycle are not positive - My CS Score and ECFMG Certification came in during Jan. 2019. I think that was a significant factor in not getting more interviews.

I will most definitely apply broadly in FM and IM, and i appreciate the advice about applying to new program. Would you recommend I reach out to new programs early with letters of interest?

I would recommend reaching out to new programs. Some offer audition rotations such as North Country Healthcare's new family med residency in Flagstaff, AZ. Also, for clarity, did you graduate in 2015? If so, what caused the delay in CS score and ECGMG certification? You don't need to be specific, but that is something that will stick out to programs so you should have prepared answer.

The delay wasn't on the CS and ECFMG end. I took CS in December, 2018 and had been under the impression (incorrectly) that Step 1 and Step 2 scores would be enough to get interviews from applying in Sept. So the delay was my own doing. I work full time, and in between doing observerships and externships, taking time off to go to a new city with poor exam dates availability proved to be difficult.

Thanks for those programs! I will keep the ACGME new program list handy for other FM/IM programs and reach out.
