Does holding a visa to the US, graduating from a US university, and graduating from a foriegn med school classify you as US IMG?

or would that make you a FMG?
 
Yes and yes.

Although to most people USIMG means you went to a 4-year US-system school (typically in the caribbean) and did your rotations in the US. The "rotations in the US" part is what helps USIMGs on match day.
 
I think what your saying is that you are not a US citizen or permanent resident, you have a student visa and did your 4 years of med school in the US or in a US accredited school. If that is so, you are a 'national foreign med student' and your residency application is through ERAS and the regular US match but your visa requirements for residency is through ECFMG where they will offer you only the J1 visa. If you want an H1 you'll probably need a lawyer to get it for you.

2. If I am a US permanent resident, went to college in the US but graduate from a foreign medical school, am I an IMG?

1. IMG = FMG; International simply sounds better than foreign. Though, the ECFMG and NMRP haven't caught on yet as they almost exclusively use the FMG term.

2. Yes. All that counts is where you did your M.D.

Yeah! I refuse to use their services until they call themselves ecimg! That'll show'em all right.
