USC Application Form?

Is anybody applying for USC?
I can not find their Application Form on their web site, although I have the informational papers.
Please guide me....????!!!!!

i was wondering if usc need sealed recommendation letters?..can't it be a copy ?..also i noticed they want original documents like the dental license in my home country! i was wondering do they need the originals of everything even for the initial evaluation?
