USC Application

Hi-
I am trying to apply to USC for an away rotation and can't seem to find the "general USC application" you have to fill out. Apparently, it is somewhere on the Student Affairs site but I can't seem to find it...anyone know?

Click on Application form PDF in the upper right corner.

Enter all your information in the fields. Then finalize the application, print and mail it in.

Only thing is that the application page seems to be down at the moment.
