Use of first name?

I was reading an article in one of the forums regarding calling a physician by their first names instead of their given title. Made me think of my shadowing experience where the doc didn't mind being called by his first name. Would an of you be offended or insist that a patient address you by doctor.

I'd prefer to be called doctor, keeps the relationship professional.

I prefer my first name but I always introduce myself as dr. firstname newfeet.
