Various employment as a healthcare worker....how to list in AMCAS/AACOMAS?

Hello all
I've been a physical therapist for over 10 years and have worked for numerous companies.
Do the different companies I've worked for need to be entered separately in the work section of the application or list it once and expand on the different places/types of patients I saw/managerial experiences in the comment section under that one heading "physical therapist"?

I'm a clinical social worker- same boat. I included everything separately, as the jobs were so diverse. (Cut and pasted directly from my resume.)

The only exception I made to that was the place I worked full-time, but also did some concurrent part-time work in another department. I put both titles into "Experience Name", and in the comment section broke down the responsibilities for each, listing the dates worked for each position.
