Anybody know if Wake Forest requires your references to fill out an assessment form in addition to letter of recommendation? I vaguely remember this to be true, but I can't seem to find the forms.

If you read the forms they have and their instructions online, your references only need to fill out the forms if you have individual letters being sent, and not the composite letter. If you need to get it, go to the instructions on how to fill out the form, and there should be a link to it from there.
