I would appreciate any advice you may give about my school list and what you think my chances are!

I would also like your input about back-up options if I am not accepted anywhere!

Surely youre more interested in advice from admissions experts/physicians/admitted students, but I'm applying now w the same exact gpa (3.86) and a 508 (130/125/126/127). Im from NY. Ive heard good things about my school selection from experts on sdn, and it mirrors your list a lot. However I applied to just 22 MD schools and 8 DO schools. You mentioned you want some back up options, and DO is a very viable option for you. Ive already got interviews at DOs with my stats. You are competitive at all DO schools, and i beleive UNECOM has a preference for people around New England. (Also consider PCOM, NYITCOM, and RowanSOM)

Definitely remove:
East Tennessee
FSU
Howard
Meharry
Buffalo
LSU (both)
NEOMED
UAB
U of A
Arkansas
Louisville
Minnesota
USA
UW
WVU
All because you are an OOS ORM applicant. All of these schools give strong preference to their IS or URM applicants. Even Loma Linda is a bad idea unless you are a devout christian. Forgive me for assuming, but it really seems like you just looked at the lowest-stat schools on MSAR and added them to your list. You’re a decent applicant, but you’ll have to apply primarily to your IS schools and private OOS schools who have no particular interest in their IS applicants. Some to add are:
Wake Forest
Temple
Drexel
VCU
EVMS
TCU-UNT
to name a few. You should also add at least 10 DO schools to your list.

Absolutely. You even added HBCU schools as an ORM. Must do some research!
