After #2, it doesn't matter much.

After #20, it doesn't matter at all.

The last time that I looked at them, they only ranked the top half of the 125 allo schools (about 62). The bottom half are all just the bottom half.

This is true. There is no complete list my friend.

Do a search, someone posted the "complete" list last year that gave the program director ratings for each school (I think on the free version, you can only see Harvard's and John Hopkins rating). If you're looking for a ranking of all schools, well - I guess you can just assume that any school that is not on the list would be considered bottom tier...
