What are some good websites to download succint (1-2 pages) patient education handouts for basic diseases encountered in the clinic? Thanks.

When I was a student, we made 1-page handouts to give to patients that provided a simplified pathophysiology of the most common conditions (HTN, HLD, DM, etc.). At the bottom there was a section where we could write in their meds/doses/how often to take. If you cant find anything then consider making your own?
 
JAMA has a patient page published in usually every issue. Hopefully this link can provide you with all of them:

If you have access to MDConsult, there's a Patient Education section that usually has nice handouts as well on a topic you searched. And sometimes, there's a bonus Spanish version on some topics as well.
