Can someone plz tell me how they r related or different. and how they r related to Transketolase enzyme and thiamine

hi! both are caused by a thiamine deficiency but the mechanism for both is a bit different. thiamine is a vitamin that is needed for glycolysis, TCA cycle, and Pentose phosphate shunt. Respectively the enzymes that need thiamine are pyruvate DH, alpha ketoglutarate DH, and transketolase. basically thiamine deficiency is a low ATP state since all these processes don't work. RBCs and the Brain can only use glycolysis so they get really messed up with a lack of thiamine.

Wernicke/korsakoff is due to CNS probs which causes personality changes and confabulation.

Dry beriberi is due to the peripheral nervous system gettin wrecked so you get polyneuropathies and muscle wasting.

There's another thingy called wet beriberi that involves dialated cardiomyopathy, heart failure, and edema. The myopathy is somewhat reversible if you catch it early on.

When you give thiamine you'll know your treatment is working if the red blood cell transketolase increases!

More like +transketolase assay indicates thiamine deficiency. Improvement of symptoms indicates treatment is working.
 
--For Wernicke's Encephalopathy, it's due to a problem of phosphorylated sugars not being able to leave the cell, causing a pulling of water into the cell and dying.

Transketolase and transaldolase are the enzyme used to remove these sugars, and require thiamine as a cofactor. If one enzyme is missing, the other will take over. This does not occur in Wernicke's area of the brain (posterior temporal lobe), where Wernicke's area only uses transketolase. So if you are an alcoholic who does not have thiamine and does not have his transketolase functioning properly, when given glucose, he will entire the pentose pathway and those cells will swell. This is the cause of Wernicke's encephalopathy.

--Also something related to this, transketolase is needed to go from ribose 5 phosphate (in the pentose pathway) to fructose 6 phosphate (in glycolysis)

--Wernicke Korsakoff syndrome is due to a problem with mamillary bodies, and you have a problem moving short term memory to long term memory

--Beriberi means the heart has given out, and caused dilation. If you have "dry" beriberi, it occurs until you have heart failure. If you have "wet" beriberi, the disease has gotten even worse and fluid is pouring into the lungs. Wet will also present with massive ventricular dilation.
