a honor or just some cerificate you have after you pass all steps? I am filling out VA applications and there is a question asked about it.

It's a certificate that used to be issued by the NBME after you've passed all three parts of the NBME examination. Then it was changed to the USMLE and the NBME did away with the certificates. So you'll see some of the older attendings (who finished in the early 1990s) have a "Diplomate" certificate from the NBME, but since then they're no longer fashionable.
