what should be included in your PS intended for toptier schools?

The same thing that you would write for "lower tiered" schools-- write about why you want to go into medicine and why you would be good physician. Write eloquently and grammatically correctly. Write in a fashion that shows that you are capable of communicating well with others and that displays that you are an intelligent person who has the writing capabilities beyond that of a 5th grader. Good luck, doctor.

I am still debating whether i should talk about my research in my ps or not....
Most people stress on the fact that personal statement is about WHY you want to be a doctor,
but for MOST toptier schools, they want people with good research experience
Not that I don't have good ones, but I don't think they really add on to my career decision (know what i'm saying? it's not like finding out that this protein helps certain process or the protein is expressed when the animal is being treated with this chemical will give me the epiphany that i want to help people nor will the specific results or process give me incomparable skills that will help me to become a great doc)
now the problem, what if they were to just read my PS and nothing else and simply conclude that i am not research material?
will they look at the whole app?

Mentioning research can't hurt no matter what schools you are applying to. I think the main thing is to focus on what research taught you, e.g. critical thinking, teamwork, time management, organization, troubleshooting, etc. These are all qualities important in a doctor and hence will fit in your PS quite nicely.
