They do not get the choice in this case, there is no consent. Private or not, it is supposed to go on the form even if deceased or unwilling. My parent's are private people and told me that they do not want me to put their information on my AMCAS application. Do I just say I am unable to provide such information? I'm sure AMCAS won't penalize me if my parents don't consent to their information appearing on my application right?
It's for the background check, socioeconomic status, and application statistics, so they don't get much of a choice. If you are not a ward of the state at some point in your life, it will be considered derogatory to not put their information. I think the only time it has not been derogatory is really if they are unknown (e.g. "ward of the court or state").
If you are currently a ward of the state or have been, there is a special form attachment that you will know from your state to submit with your application as the legal guardian even if there is a personal guardian. They are accepted in lieu of parental identification.
