When a job listing says "call 1:4" does that mean 1 in 4 nights or weekends?

Usually means weekends.

But the definition of call is very different. Some groups consider working 4-12 PM (and having morning off) "a call" and some groups consider it a normal working day but different hours.

Caveat that DR call often also includes weeknights as well. It's rare, but someone needs to be the name on the call list to do, say, a pediatric fluoro exam for intussusception or midgut volvulus or the rare procedure that can't wait until the morning.
 
I often think that means call is divided among four people but as people say call can mean a different things to different groups.
