Do most of them? Anybody have a list of the ones that do?

All of them do, i.e. 1 year IM + 3 year Neuro. Most residency programs would arrange the 1 year IM for you at the same institution (categorical spots), while others require you to arrange it by yourself (advanced spots).

Very helpful. Do you know which programs (the minority) do the advanced spots?

All neurology programs require a prelim medicine year. There's no way around it.

Which neuro programs will find you a IM prelim and which programs will require you to find one yourself?

You need to check Freida. I doubt anyone has the time to list them all for you.
