You could talk to the hospital supervisor and say you're applying to medical school and it would be great to get a letter of recommendation from her. You could then say you understand that there hasn't been much contact between you two, but that you've been working closely with the secretary and perhaps she would be able to give the supervisor some information about your character/work ethic/etc (obviously make sure that the secretary is okay with this first but it shouldn't be a problem).
