My friend who had his shin bone fractured was given pain killer shots in the hospital. He said he was having pain turning so he wanted the injection in his arm. I know he said that because he didn't want to strip in front of the female nurse.

But the doctor insisted on giving it in the buttocks.

Is there any particular reason for this insistence?

The location & method of drug delivery is based on the characteristics of the drug.

However, you're asking for advice relative to a specific individual, which, I believe is a violation of tos.

You'd have better luck with an answer if 1) you don't refer to a specific individual & 2) have more specifics about the drug.
