Position Summary:
The Clinical Pharmacy Specialist - Coumadin Clinic will manage the anticoagulation therapy for patients prescribed Warfarin (Coumadin) to ensure the safe and effective use of the drug and minimize complications associated with such therapy. The Clinical Pharmacy Specialist's responsibilities include, but are not limited to: assessing the patient's drug therapy and medication history, providing point-of-care assessment of the patient's anticoagulation status utilizing the I-Stat system, adjusting Warfarin dosages, as appropriate, to maintain the target level of anticoagulation, providing patient education on Warfarin, including potential drug-drug and drug-food interactions, documenting patient care activities in a progress note in the Computerized Patient Record System (CPRS), documenting pertinent patient data in the CoumaCare database, performing activities associated with scheduling patient appointments, when necessary, referring patients who experience adverse drug reactions.
Location Summary:
Shreveport is located in the northwest part of Louisiana, near Texas and Arkansas. Shreveport is the sister city of Bossier City. The two cities are separated only by the Red River. There is an excellent climate & low cost of living. We have numerous colleges and universities, theaters, lakes, golf courses & and other available family entertainment. Sporting activities include hunting, fishing, a semi-professional hockey team, Battle Wings arena football, Shreveport Sports minor league baseball and the Independence Bowl. Cultural activities include Sci-Port museum for children, Norton Museum of Fine Arts, Marjorie Lyons Playhouse, Peter Pan Players, Strand Theater and Century Tel arena where numerous concerts are held (i.e. Aerosmith, George Strait and Disney on Ice, etc.).
Position Requirements:
(1) Must be a U.S. Citizen; (2) Graduate of a degree program in pharmacy form an approved college or university. The degree program must have been approved by the American Council on Pharmaceutical education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP) OR Graduates of foreign pharmacy degree programs meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. This finding may be based on any of the following: (a) a letter of acceptance into a U.S. graduate pharmacy program recognized by the ACPE; (b) Written certification form the Foreign Pharmacy Graduate Examination Commission, that the individual has successfully passed the Foreign Pharmacy Graduate Examination; or (c) a letter from a U.S. college or university with a pharmacy degree program recognized by the ACPE stating that the individual's foreign pharmacy degree has been evaluated.

I'm not sure I'd make a big shift to work at a Coumadin clinic with Exanta coming out. It may just revolutionize anticoagulant therapy (if the LFT problem doesn't do it in). If you already live there, or it doesn't require a huge change in your lifestyle, then go for it.
 
running a coumadin clinic is probably one of the most boring jobs you could possibly do. I did a 6-week long rotation running one, and I won't recommend it, unless you want an easy A for your transcript. you check INR's and then find out that the reason the patient is 2 points higher than they should be is because they drank a 6-pack last night.

Exanta should be released late this year, maybe early next. that transient increase in LFT's goes away whether or not the medication is dc'd, so the FDA will probably not hold it up. it has it's problems, though; like if you miss one of the twice daily doses its effectiveness is essentially nill, it's not as easily reversible as the other anticoagulants....so, the coumadin clinic might be around for a while, yet.

But boring as hell and you only deal with one drug. I know some people who do coumadin clinic 2-3/week and then other stuff on the floors, which is better, but I still wouldn't reccommend it. You deal with only one drug, your skills with other drugs rot, so no I would not reccommend it. Stay away from there, go grab a more broadly oriented clinical job. Best of luck.
