I feel like there needs to be one about being an intern/resident and after disagreeing with your attending (and thus having to do what they want instead of what you want), a consult or a lab proves you were right and they were wrong. I can't think of something right now, hopefully it will come to me.

These are ****ing amazing. I really like a lot of the Intern ones.

At my program we're not allowed to moonlight though so those achievements are locked for me. Sadness ensues.

Here are a lot to add (nights are slow):

05G - Not Your Fault
As an intern, pronounce a patient death.

15G - Not Today
Successfully resuscitate a Code Blue.

10G - Clavien Didn't Do Nothing Wrong
Present a patient complication or death at M&M.

05G - Trust but Verify
After a student has already presented their H&P to you, ask a patient a question that provides more answers about the HPI.

15G - No Hitter
Make it through an entire call shift (minimum 12 continuous hours) without a single consult.

50G - Perfect Game
Make it through an entire call shift (minimum 12 continuous hours) without a single page. Hint: Requires a lot of active management of nursing staff, or a small census

100G - White Cloud
Achieve your 3rd Perfect Game of the month.

05G - If You Want a Job Done Right...
Successfully tell a nurse to place an order under your name.

05G - ...You Gotta Do It Yourself
Perform a task that normally falls under nursing responsibilities at your institution (examples include changing IV infusion rates, placing routine Foley catheters, starting IVs, recording ins-and-outs, etc.)

05G - What's That On Your Shirt?
Accidentally break the field while performing a clean or sterile procedure, necessitating a change of glove and/or gown.

10G - But Baby It's Cold Outside
Have your first patient leave AMA.

05G - Work Smarter Not Harder
Have your 100th patient leave AMA.

05G - Uphill Both Ways
Listen to an attending lament duty hour restrictions.

30G - How Come The Other Services Cap?
Write 30 or more daily progress notes in a single day.

10G - I'm Not Even Supposed To Be Here Today
While on home call, get called in to see a consult.

05G - On The Company's Time
Have a bowel movement in a hospital bathroom.

10G - Knock On Wood
Watch a feature-length film with a minimum runtime of 100 minutes from start to finish in one shift (minimum 12 continuous hours).

05G - Slow Night
Watch a feature-length film with a minimum runtime of 100 minutes from start to finish without interruptions in one shift (minimum 12 continuous hours).

05G - Prepper
Perform a bedside procedure from start to finish without needing to call for extra supplies.

05G - Circadian Rhythm? What's That?
While on nights, sleep for more than 4 hours total during a single shift.

15G - Pass The Buck
Successfully transfer a patient to another service.

05G - Tag! You're It!
Have a patient transferred to your service from another.

05G - Case Report
Witness an attending say, "I've never seen that before."

05G - Performance Anxiety
Witness an attending say, "That's never happened before."

10G - Good Riddance
Have a patient fire you as their physician.

05G - It's Not So Bad
Eat a meal from the hospital cafeteria.

05G - I'm Really Sorry But...
Call a consult that your senior does not want but your attending does.

05G - ...This Comes From The Top
As a consulting service, field a consult where the requesting service indicates that it was an attending who wanted the consult placed.

05G - Did You Even Look?
Get a call from the floor asking for prescriptions in the patient's chart for discharge, and when you look in the paper chart or EMR discover that they were present all along.

15G - Heisman
Successfully defend against an admission to your service from the Emergency Department.

10G - It Ain't Much...
Start a shift with 5 or fewer patients on the census. (Consult census does not count for or against this achievement.)

15G - ...But It's Honest Work
Make it through an entire shift (minimum 12 continuous hours) without letting the census exceed 5 patients at any given time. (Consult census does not count for or against this achievement.)

10G - Preventive Medicine
At any single point in time have an inpatient census of 0 patients. (Consult census does not count for or against this achievement.)

50G - The Game
Signout an inpatient census of 0 patients. (Consult census does not count for or against this achievement.)

10G - The Hypocritic Oath
Have a Jehovah's Witness patient accept an albumin infusion.

05G - Doxximity
Someone has discovered your SDN username.

05G - Evidence Based Medicine
Witness an attending direct care using outdated recommendations.

10G - They Grow Up So Fast
Have a medical student place an order under your name.

05G - Correct The Record
Make an addendum to a note you wrote after an attending has already cosigned it.

10G - It's Medicinal
Write a prescription for marijuana.

05G - When It Rains…
Field 3 consults in a single shift (minimum 12 continuous hours).

05G - …It Pours
Field 3 consults within an hour.

05G - Alpha Omega Alpha
As an Internal Medicine resident, field a consult from Orthopaedics for management of hypertension or diabetes or acute kidney injury.

05G - Day Shift Problem
As an intern, while on night shift, neglect to do a task signed out to you by the day team.

05G - Did Anything Happen Overnight?
As an intern, while on day shift, independently discover that the night shift intern did not perform a task you signed out.

05G - No Referrals Necessary
As an Emergency Medicine resident, have a consulting service decline to see the patient and state they will schedule the patient to be seen in clinic.

25G - Free Agent
Attend undergraduate, medical school, residency, +/- fellowship all in different states.

15G - Journeyman
Free Agent achievement plus attending position in a different state.

05G - Wellness
Attend a resident retreat.

10G - Eligible Bachelor/Bachelorette
Remain unmarried from the beginning of medical school through signing your first attending contract.

25G - Through The Cracks
Successfully match into a categorical position after failing to do so as a US senior. There is no time limit after graduation for this achievement.

05G - New Purpose
After failing to match as a US senior, SOAP into a program during the same cycle.

I added instances where I've unlocked some of the achievements I've come up with.

10G - Independent Learner
Formulate a plan based solely on a medical student's History & Physical and have an attending sign off on it. (Findings of sensitive portions of physical exams that cannot be performed by a student alone do not count against this achievement.)

05G - Matters To Attend To
Miss all or a portion of signout to retrieve food delivery that you ordered.

05G - Dereliction of Duty
While on Labor & Delivery, have the Emergency Department send up a pregnant patient to triage with a non-obstetrical complaint.

15G - "Eww! Women Parts!"
While on Labor & Delivery, have the Emergency Department send up a patient to triage with a negative urine pregnancy test.
Happened on Sunday, patient had peed herself at home and boyfriend thought her water broke; patient claimed she was 5 months pregnant and her belly was flat as a board. I'm a terrible sonographer and even I could see a nice endometrial stripe and no IUP or ectopics in her abdomen. UPT that we drew was negative

10G - They Graduated YOU From Medical School?
Correctly diagnose a patient's disease after they have been incorrectly diagnosed by the Emergency Department.
Diagnosed a patient with B12 deficiency as an M3 after he had been seen in the ED for paresthesias in glove and stocking distribution and referred to neurology without even so much of a B12 level.

05G - Not As I Do
Spend your 100th dollar at hospital vending machines.

Thought of another achievement that I complete not infrequently.

05G - Street Rat

Eat a snack item from the “Patient’s Only” pantry (examples include sandwich, saltine crackers, chips, etc.)

The next one was inspired by an attending who would often ask if a patient would finish [food item] on their tray:

10G - Scoundrel

Eat a food item that was originally part of a patient’s food try.

And the following I have never seen happen but that’s why it’s an achievement, right?

30G - One Jump Ahead

Eat an entire patient meal.

Another hidden achievement obtained:

15G - Independent Practitioner
As a resident or fellow, disregard an attending’s [non-evidence-based] plan for a patient and do what is best or evidence-guided for that patient instead.
