Try US news & world report primary care rankings.

In my opinion:

OSU-COM & TCOM are the best 2 schools

Never woulda saw that coming....

Top schools, IMHO, include: msucom, osucom, unt, ccom, pcom and umdnjcom. all of them are "good," per-se...
take a long look at match lists, curriculum descriptions, clinical rotations, and hospital affiliations and come up with your own. what we think doesn't matter much to you.
location may also affect such a list, but personally, as long as its not in the sticks, i dont really care.

I found some data on usnews from which I calculated a competitive score for schools that responded to usnews and provided data. Competitive scores are obviously not an indicator of how good the schools are, just how hard they are to get into. I thought there might be a limited degree of correlation, however, so I'm providing these data. I read somewhere that the competitive score is calculated thusly:

CS = GPA*10 + MCAT(combined)

The max is 4.0*10 + 45 = 85

The highest score I found for average matriculants was Washington University with average competitive score of about 75.

As for D.O. schools:
format: school, average competitive score, average gpa, average combined MCAT
