I am collecting contact names & addresses for verifying volunteering, and realized that I do not have the contact information for a camp that I helped run during the summer a couple of years ago. ... Local sports camp made up of primarily volunteers -- mostly dads and moms. I'd hate to lose the 40 hours of volunteering... any ideas?

Contact info isn't mandatory for all activities.

There's no reason not to list the activity anyway, though you could perhaps list any other volunteer you knew as a contact to verify the activity. The honor system still counts for something in the med school application process, if you can't think of anyone.
