How do you know when to send it? Will schools email you to request it? Does it delay you being marked as complete? Can you request your undergrad to send it ahead of time? Haha sorry so many questions!

Three that I applied to and needed them were Temple, Tufts, and BU (I know already stated) but I believe it was Tufts that contacted me around January of the following year asking for the letter instead of immediately after the secondary was submitted.

It's an institutional action. In other words, any time that the UG school you attended took disciplinary action against you in any shape or form—whether it be a simple write-up or academic dishonesty. However I would hold off on reporting them until you are positive that it's recorded in your transcript or by the Dean of Students at your school. I had one, so I went over to the DoS office and she confirmed that I had one on file.
