Like if I volunteered at a hospital 3 years ago (sophomore year) for 300 hours will medical school see this experience as too... "outdated"? What's the maximum number of years that can pass before I should renew my HCE's?

That's not exactly how it works. Anything completed after high school won't expire. However, if you did something for a year and then stopped doing it, adcoms could say, "Well, maybe this wasn't too important to them. After all, they chose not to continue it" - so it looks more like checking a box

I'd hope you'd do something medically-related in the three years before you apply.
