In the 2019 applicant guide, it says;

"A first generation college student indicator of "unknown" indicates:

- parents who completed their highest level of education outside the U.S"


They list other reasons as well, but I am a bit concerned about this one. My parents only completed high school, but it was in another country.

I am quoting a portion of your message to put my response in context.

You can list your parents education and occupation on the application. Adcom members can read and draw their own conclusions even if you don't meet the criteria for a given "check box".
 
Thank you for confirming that. I was just wondering if medical schools will be able to know this, seeing as how they will mark my first generation status as unknown?

There is a place on the app to list their highest level of education, likely right next to the first gen status.
