Neurology has really good DO match rates, If I remember correctly. Like 90 percent last year. I am not sure about fellowships, but you would just need to try and match into a spot with good fellowship outcomes in the past. Preferably in the same institution. This is what I was told for my interest in Nephrology.

That's because when you do internal medicine-> pulm & crit care-> you only need an additional 1 year of your fellowship to add on sleep medicine, but you can do sleep medicine without pulm & crit care (I work in a pulmonary department, and the physician I worked for was pulm & crit care, and I went to department meetings with pulm, crit care & sleep)Most sleep specialists are pulm
N=1 but my husband’s neuro/ sleep doc is a DO!
I have a family member who saw a sleep/neuro doc who is a DO, so N=2N=1 but my husband’s neuro/ sleep doc is a DO!

Sleep isn't competitive. Neither is neuro. IM-> Pulm is moderately competitive but you don't need to be some world beater student to get it.
