I think it's okay for the most part. You can put down as many as you can remember.

Certiphi also fills in the blanks -- I didn't put down my college addresses and it added them in my official report.

This won't matter. At worst the school will email you and ask you to send then the list if it looks sparse on Certiphi. This isn't going to get your acceptance rescinded or anything.
