You should contact the dean of students office, head of dining, or director of housing / dining to ask them for a letter explaining what happened.

Did you explain the IA in your primary or just write that it was removed?
 
Someone at your school will be familiar with this particular type of infraction, the appropriate response, and the resolution of the incident. Honestly, this one isn’t hard. Contact the individuals I listed above, explain what happened and when, the outcome, and that medical schools need additional information. The officials will be able to write this up and include that it was not recorded in your file. It’s pretty simple.
 
If there isn't anyone to detail the situation though, should I just have the dean's office send a letter basically saying that?
Again, this is a fairly common infraction. Just because it wasn’t included in your record does not mean they can’t explain what happened. Someone at your university will understand what happened and can write to that effect. You don’t want to be seen as being evasive.
 
Any thoughts on what I should do? I guess I don't have many options but I was wondering if I should email the school about it first or just send the letter, which makes it look like I'm trying to evade it since the school won't say anything about the situation @tantacles @gonnif @GreenDuck12 @Goro

I emailed the Dean's office and they replied saying "it seems you have already received a background check letter which does indicate that you have no history to report. Please understand that we do not release any documents in our files to anyone. We only summarize in letter form a student’s reportable disciplinary history and only with their written authorization will we send this letter to any third party."

Unfortunately my sanction is not reportable since: Reprimand: reportable for one (1) year unless it was active when a Censure was issued.

So basically it seems like they will only tell the medical school that I have nothing to report, but won't go into detail about why (files destroyed due to nature of sanction) there's nothing to report. Should I still agree to have this letter sent, or should I just ask the school first? I did say in my primary that the file was destroyed so I'm not sure what the school would be expecting

Send the letter. If they ask for further information, you can forward the e-mail your school sent you as an indication of good faith.
