Where should I go this summer?

Whichever will result in publications is the better opportunity. It is important to work with a PI who is prolific in pumping out research. For IM, the type of research matters less.
 
I am MS1 Currently at a low tier/únranked academic medical school. I was accepted in a summer internship at MDACC doing basic science research in cancer biology.

Should I go to the research summer program?
or should I stay at my school doing research for the opportunity to followup on the project in the second year?

I am interested in IM maybe cards or GI in the futurere

This is the way.

Regardless of output, the 1st summer of medical school is a valuable opportunity to experience something new and interesting. If you can get a publication out of it, the better. However, I really encourage everyone to just go with what they want to do early on in medical school. You will have plenty of time to follow-up on projects.

There is no reason why you can't follow-up on projects at MD Anderson after the summer either if your data collection is done. Also, food in Houston is pretty dope.
