Did you apply for the INOVA campus? You could get the best of both worlds being near DC for rotations. I believe you can email them and ask for consideration for INOVA if anyone wants to switch campuses. All rotations would be there. There is also a lottery system for rotations so if you stay at Charlottesville campus, you can try to arrange a switch with another student for their INOVA rotations. Charlottesville itself is a liberal hub and the qMD group is actively and friendly, but they also go for Pride at D.C. Charlottesville has a transgender clinic. Many people also do MSSRP over the summer for a summer research project.
Cornell sounds like it would be a better fit for you though if you get off the waitlist for all the reasons listed.

Sounds like in terms of PTE, seems like you're leaning UVA and the cons of Georgetown seem pretty significant compared to UVA, so I'd go UVA.
Of your WL options, sounds like you really want to be in NYC and would thrive there, so best of luck getting into one of those. Of the two, sounds like Cornell might be a better fit for you than Columbia based on the culture and location within NYC, and you didn't mention this in your pros but there's also the fact that you mentioned you identify as queer and they have the Wellness Qlinic (nation's first LGBTQ+ mental health clinic) and you also mentioned being interested in refugee projects and health justice and Cornell also has the huge Center for Human Rights where they work with refugees and asylum seekers.

Between UVA and Georgetown, I'd probably go UVA as it seems like you like it more. Both have strong surgery departments so you'd be fine in that regard. I can't speak to psychiatry.
That being said, send an LOI to one of the NYC schools and see how things shake out on the waitlists. Either way you're in a very good position.

Thank you so much everyone for the advice. You’re right - I have been leaning heavily toward PTE’ing UVA, and have already submitted letters of interest to the NYC schools. If I get into one of them, I’ll probably go. I appreciate y’all helping me think through this!
