I'd personally go with Columbia because the name is bigger outside of medicine (which could help if you move away from clinical), and if you have any interest in NYC.

I would never want to have roommates so I’d go WashU or find a way to get some sort of loan to cover higher rent. The only thing is that outside of collateral based and educational loans, the rates aren’t worth it.

You're fine either way, what an awesome choice you have...wouldn't underestimate how nice it'll be to have your own place with space tho. In St. Louis, you can legit live in a very nice apt for a fraction of what a mediocre apt will be in NYC. NYC is NYC, but STL shouldn't be slept on. Also, would ignore the idea of name outside of medicine, unless you plan on starting a company in the Northeast or something lol

Lastly, I think this is one of those situations where you can legit just follow your heart. If you're leaning Columbia, take that! They are equivalent institutions and unless the COL is a major con, then you may regret not going to Columbia if you're leaning towards it. Good luck!

You'll be fine either way. "Only game in town" makes 0 difference for you as a medical student, I promise you this.

But man, you get to live for 4 years in NYC for free effectively? Hard to beat if your dream has been to live in NYC since you were a kid...
