It just depends on your goals. Why are you doing the MPH?

Also, if you're interested in a highly competitive specialty, this will probably be an ideal time for you to get some research in on the side.
 
Neither OB/Gyn nor IM "require" research, and it won't be "expected." If your long-term goal is academia, then establishing a track record of publishing would certainly be helpful down the road, but not necessary for your residency applications by any stretch.
 
Depends on your intended specialty. For competitive specialties, it is still expected that you are productive regardless of whether it's a research year or a dual degree year. So you should try to stay active in research and to publish. If in a non-competitive specialty, then it doesn't really matter.
