Be aware that most public/state schools have strong in state bias, which will severely limit your odds of acceptance.

I would focus primarily on DO schools unless you retake and get a higher MCAT

If you apply MD, avoid Cal northstate and OOS unfriendly state schools

With that MCAT and considering the state schools that CA has, it likely would be best to apply DO only.

Fill out the template. And yes your MCAT crosses all MD off your list unless there are extremely unique circumstances you haven't disclosed.
