You definitely have all the basic ingredients for a solid application, and you can be competitive at most places. Not sure if the lack of clinical volunteering would be an issue at specific md/phd programs (I doubt it'll be an obstacle at most programs given the rest of your clinical and volunteering); I'll let someone else comment on that.

Most important thing for your md/phd school list is what do you want to research in your phd? I think sharing that would help others provide potential names. Research fit should be among the top considerations for making a list, start going through programs and seeing if they have departments/labs/programs in your area of interest
